Aldactone

Dosage

• For hypertension, the typical starting dose is 25-100 mg once daily, which can be titrated up to 50 mg once daily as needed.
• For edema due to heart failure, start at 25 mg once daily, increasing to 50 mg once daily if tolerated.
• For edema due to cirrhosis or nephrotic syndrome, start at 100 mg daily, range is 25-200 mg.
• For primary hyperaldosteronism, use 100-400 mg daily pre-operatively or as long-term maintenance.

Indications

• Edema due to heart failure, cirrhosis, or nephrotic syndrome.
• Hypertension, often used with other blood pressure medications.
• Primary hyperaldosteronism.
• Acne and excessive hair growth in women.

Contraindications and Precautions

• Avoid in patients with hyperkalemia or severe renal impairment
• Use caution in patients with impaired renal function, as Aldactone can cause hyperkalemia, hypotension, and worsening renal function
• Monitor electrolytes, especially potassium, and adjust dose accordingly
• May cause gynecomastia, especially with higher doses
• Avoid in pregnancy if possible
